Students placed on financial aid suspension may appeal this decision if they had extenuating circumstances that prevented them from meeting the Satisfactory Academic Progress (SAP) requirements. All applicable steps MUST be completed in order for your appeal to be processed.

STEP 1:  Please explain semesters on your transcript where you withdrew or were unable to successfully complete your courses.

• If this your first time appealing, please explain ALL semesters with courses you did not complete. 

• If you have previously submitted a SAP appeal at QCC that was approved, please explain any new semesters that were not previously addressed.

1. What circumstances affected your ability to be successful?  Please limit your answers to the specific circumstance(s) that affected your
academic performance the most.  

2. How has each circumstance been resolved? What have you put into place to ensure you are academically successful going forward?  Please be specific.

3. If you have previously received an appeal approval at QCC, you must provide documentation of the circumstances you described above.

Documentation that those circumstances have been resolved may also be required, particularly for medical circumstances. Provide additional information by uploading it to the Financial Aid Document Center on The Q. If required, your appeal will be recorded as Incomplete and will not be reviewed until documentation is received.

STEP 2: If “FAILED” is listed above for the Met Maximum Timeframe Criteria, answer the following questions:

Students who are not meeting Maximum Timeframe cannot complete their program within the number of credits allowed by the federal government.
Often, courses taken from previous programs are factored in as electives.

You must be actively enrolled in the degree or certificate program for which you are appealing to receive financial aid.  If you are not appealing for
the major(s) listed below, you must change your major before a decision can be made.

What Happens Next?
· Appeal decisions are made on a case-by-case basis. Depending on the time of year, the review process can take anywhere from 6 to 12 weeks. If you submit an appeal that is incomplete, the process will take significantly longer.
· Additional information or documentation may be requested for any appeal.
· If you choose to enroll in classes while you wait for your appeal to be reviewed, you are responsible for:
          o attending your course(s)
          o acquiring required books and supplies
          o your academic success in these courses
          o reviewing your degree audit to ensure that your course(s) fulfill requirements for your current program. Courses that do not apply may not be funded.
          o payment of your semester charges
· We are not able to consider your appeal for a current semester if you withdraw, fail to attend, or do not earn a passing grade in any course at the time it is reviewed. Financial Aid semesters are Fall, Intersession/Spring combined, and Summer I/Summer II combined.
· Please note that appeals must be approved before the last day of classes to be considered for financial aid for that semester.
